Sure! Picture it this way: spacetime is all hyper-bendy all the time. It’s always flopping around like a flag in the wind, just going haphazard. Once in a while, two points in spacetime will get the idea that they want to hook up and form a link for a bit, usually because there are massive amounts of energy in both of these points. So they reach through themselves and find one another and create a brief link. Put simply, a wormhole is a hyperlink from one point in s/t to another. See the diagram.
